In The Sixth Just Leave It Blank, You Don`t Need To Put In Anything. phew.

Next Is Your Gender Left Is Male Right Is Female
Lastly Put In The Verification Letters/Numbers. Make Note That If A Letter Is In Capitals You Put Capitals (Yessh It Is Case Sensitive)
